Encounter Notes for a Smooth Transition

At MDH Insight, our EMR PDF Note system is something that truly sets us apart from the competition. Most legacy EMR vendors do not provide the encounter note as a part of the export, creating a challenge for many practices. At MDH Insight, however, when clinical data cannot be migrated to the new system through normal channels, we can create a PDF document that summarizes the clinical notes from each encounter to attach to the patient record in the new system.

An EMR PDF Note typically includes a summary of the encounter with the following information:

  • Date of Service
  • Attending Physician
  • Medications
  • Problems
  • Immunizations
  • Allergies
  • Family History
  • Social History
  • Surgical History
  • Procedures
  • Plan/Assessment
  • Physical Exam
  • Lab Orders

Through our EMR PDF Note process, the encounter note is a complete summary of the entire office visit for that patient. Safeguarding this summary is vital for practices to have access to past encounters when migrating to a new system. MDH Insight values the importance of these summary PDF documents and thus offers this practical, unique service to our clients.

 One Document Per Encounter

Unfortunately, many customers in the EMR data migration process expect a PDF summary and are extremely disappointed to learn that the legacy vendor does not provide it. The legacy vendor typically offers scanned documents, which includes things like x-rays, lab results, referral letters, faxes, insurance cards, and a driver’s license. However, sorting through these individual documents costs the practice money, time, and energy, as they are forced to open each one separately.

For that reason, the benefit to the summarized note cannot be overstated. At MDH Insight, we create the EMR PDF Note to be one document per encounter, with all of the pertinent informationincluded in text format. From accuracy to speed and from ease of access to organized summaries: We Deliver.
